There were two medications that were expired.

During investigation it was found, a child was able to open the door to the playground and was unattended for less than a minute.

Five out of ten children's files reviewed did not have a health statement on file.

During the investigation, it was determined that a child had been left unsupervised on the playground for three minutes before being found and returned to the classroom.

A injury occurred to a child, and the parents were not notified.

At time of inspection, a toddler caregiver did not wash hands after conducting diaper change and removed gloves. At time of inspection, a toddler class did not hand wash children's hands before snack time.

During the course of the investigation, it was confirmed a child was left unsupervised in the playground on 4.5.24.

A caregiver at the operation did not use appropriate discipline methods when they hit a child in the hand to stop them from fighting over a toy.

At time of inspection, a toddler teacher did not know how many children they were responsible for during nap time.

The director failed to ensure that the toddler classroom was within ratio at the time of the incident subject of this investigation. The director was notified that the classroom was over ratio and failed to send a second caregiver to bring it into compliance for a full thirty-four minutes, during which time a child was injured while in care.

At the time of the incident under investigation, there were nineteen children with a specified age group of three years of age with one caregiver. The child-to-caregiver ratio for this specified age group is one caregiver to fifteen children. Toddler class was out of ratio for thirty-four minutes.

A toddler caregiver failed to communicate with the incoming caregiver and administration of a child injury that occurred while caregiver was supervising children alone in the playground.

At time of inspection there was a caregiver with no active background check. This was corrected at inspection, caregiver left operation.

Based on information gathered, it was determined that caregiver yelled and pulled child's arm and slapped the child's hand, while attempting to remove the child from the sink area. Based on information gathered, a caregiver dragged a child by his arm. Note: Both caregiver's were reprimanded prior to inspection.
